Section I - Applicant Information

This document is the approval form for unmanned aircraft system (UAS) flight activity at SUNY Cortland. This form and supporting documentation should be submitted to the UAS Review Committee (UASRC) a minimum of 10 days prior to flight activities. The UASRC will review the form and documentation within 5 days. Further guidance on supporting documentation and submitting this form is provided on Sections IV and V.

Important notes: One day before flight activity, the pilot in command/operator must contact the Cortland County Airport to specify the date and time of flight plans. The contact number for the Cortland County Airport is 607-745-5872. Additionally, injuries, fatalities or property damage related to fight activities must be reported to the EH&S Office by calling 607-753-2508. When this form is approved, you will receive an email with instructions to add the date/time of contact with the Cortland County Airport to your flight activity form.

Aircraft liability insurance should provide no less than two million dollars per claim and two million dollars in aggregate. This insurance policy shall name the State University of New York as an additional insured and shall designate the State University of New York as the loss payee. Moreover, depending on the circumstances, Workers Compensation and Disability Benefits Coverage and Business Automotive Liability Insurance may also be required.

Section V - Flight Activity Agreement

The undersigned acknowledges that all information in this form and supporting documentation are accurate. Additionally, the undersigned agrees to comply with FAA regulations, and expectations as outlined in SUNY Cortland’s Small Unmanned Aircraft Systems (Drone) Policy.
